Our forums focus on current diabetes related topics that impact First Nations People. You will hear from experts in the field of research and clinical practice who will share their learnings and provide advice to assist you in supporting your clients.
The discussion aims to provide you with the knowledge, relatable guidance and practical support and tools, to help you support Aboriginal and Torres Strait Islander people living with or at risk of diabetes.
 
In this session we will hear from Cassandra Stone, Nurse Practitioner Nephrology, Master of Nursing, Galangor Duwalani Primary Healthcare. 
 
Cassandra Stone is a highly experienced Nurse Practitioner specialising in nephrology. With a passion for improving health outcomes in Indigenous communities, Cassandra has been instrumental in leading nurse practitioner-led clinics focused on chronic kidney disease, diabetes, and heart failure. Her innovative approach to integrated care has resulted in significant improvements in patient outcomes and healthcare resource utilization. Through her dedication and expertise, Cassandra continues to drive positive change in renal disease management and community healthcare
